Come on, Dr. Wu
Chapter 302: Sudden Case
Chapter 302: Sudden Case (asking for monthly ticket)
It’s not all because of you!
They suddenly discovered that Wu Xiaofu had such profound attainments in stimulating potential.
After all, if Wu Xiaofu hadn't set this precedent, the style of this competition would not have deviated so much.
Now, those doctors who previously thought they had good scores are now completely desperate, and their rankings have dropped by more than ten places.
It wasn't just Chu Jingming and his colleagues who were feeling depressed. Many doctors, including the doctor from Country J, were looking at Wu Xiaofu with a hint of resentment. The ones before him were just ranked low, but they were directly seduced and got zero.
Big zero!
Their world surgery competition has come to an end.
This night, it can be said that some were happy while others were sad. However, Wu Xiaofu didn't care about that. He only knew that China's performance had really improved.
The fifth day was the orthopedic competition.
In the medical field, there has always been a saying: ophthalmology is gold, surgery is silver, dental department is a beast, obstetrics and gynecology department works hard, internal medicine department is annoying, nutrition department eats and drinks for free, pediatrics department is noisy, psychiatry department is the leader who gets beaten and bullied, infectious department is the only department that has no way out, and emergency department will never go to the hospital.
Although this jingle cannot fully cover the characteristics of the hospital departments, it is generally correct.
If we look at the order of making money, ophthalmology is undoubtedly the first. You can see when you go to the operating room that there are a constant stream of ophthalmology patients every day, and the treatment does not need to be too complicated. Just a cataract is enough to make the ophthalmology department's income reach the top of the hospital.
Silver Surgery refers to the earning power of surgery, which is second only to ophthalmology, because the income of surgery includes surgical fees. As long as you are not afraid of death and hard work, there is really plenty of money to be made.
Among surgical departments, orthopedics is the most profitable.
Because orthopedics has the most medical devices, whether it is steel plates used in trauma surgery, metal joints used in joint surgery, or nails and rods for the spine, all of these are counted in the tens of thousands.
The surgical fee of an orthopedic surgeon will naturally not be low.
Compared to orthopedics, general surgery and urology really just earn their hard-earned money.
Of course, that was before. The situation is very different now.
After the New Year, the country began to carry out medical reforms and introduced a centralized purchasing system. Both medicines and medical devices are purchased centrally, and prices have been directly reduced to 20% to 30%, or even lower.
For patients, this is naturally a good thing. After all, after the policy was implemented, surgeries that once cost tens of thousands of dollars can now be done for a few thousand dollars. Of course, we don't know what the quality of the equipment is like.
Wu Xiaofu usually doesn't pay attention to orthopedic matters, but he heard that the income has shrunk significantly and they have started to exchange quantity for performance, which shows that orthopedics are not having a good time either.
And the original silver hammer has now become a real carpenter.
As we all know, orthopedics involves a lot of hammering and banging. You can walk around the operating room, even if you don't go in. As long as you hear clanging sounds, it must be in the orthopedics department. After all, surgeries in other departments generally don't make such a lot of noise.
This is no exception during competitions.
Look, the clanging sounds from the screen have been heard clearly.
To be honest, that operation would make even a doctor feel a toothache.
It really made a clanging and banging sound.
Boom boom boom!
A strange sound seemed to appear in Wu Xiaofu's mind: eighty, eighty.
China's orthopedics is quite good.
In terms of difficulty, the most difficult surgery in orthopedics is undoubtedly surgery involving the spine, and the spine is the most important among them. Basically, only the top orthopedic hospitals can carry out this operation smoothly, followed by the lumbar spine.
Most of these cases are caused by traumatic car accidents.
Therefore, spinal injuries are usually accompanied by other fractures and trauma. Therefore, the duration of orthopedic surgery is generally not too short and is counted in hours.
Especially for spinal cord removal and lumbar spine surgery chosen by these outstanding young doctors, it will take a long time.
Don’t think that orthopedic surgeons can only do rough work like carpenters.
This surgery on the spinal cord and spine is really a delicate job, and it is thrilling for people to watch. After all, if the force is not used correctly, it may cause these patients to be paralyzed for the rest of their lives.
There was no orthopedic surgeon in the six-member team from the First Affiliated Hospital of Donghai this time. Wu Xiaofu and his colleagues were in a relaxed mood. As luck would have it, the three orthopedic surgeons from China, two from Group A and one from Group B, finished early.
And the scores are not low, basically above 140 points, and the highest one is 154 points, so maybe you can reserve a place in the top 30.
Wu Xiaofu and his friends left early after watching the match between Group A and Group B.
It has to be said that orthopedic surgery is not as entertaining to watch and is less popular. When Wu Xiaofu and his friends left, many people had already left the observation room.
As the game enters the later stages, this is really a halftime break for orthopedics.
The neurosurgery and hepatobiliary surgery that follow are the real highlights.
Starting from the sixth day, we have neurosurgery operations. In terms of technical content, it really has to be neurosurgery.
The treatment is good, and the operating time for neurosurgery is only three days.
The same six groups are operated on in three days, which is a treatment not available in other specialties. Anyone who sees it will be envious. However, the operation time of neurosurgery is indeed not comparable to that of other specialties.
Neurosurgery is usually divided into four levels: level one, level two, level three, and level four, from easy to difficult and from small to large risks. The specific level depends on the instruments used in the operation and the difficulty of the operation.
Level 1 and 2 surgeries are more common, such as suturing scalp lacerations, scalp debridement and suturing, and some skull fracture surgeries. Level 3 surgeries include skull repair and some superficial brain tumor surgeries, while level 4 surgeries are relatively difficult and risky surgeries.
Nowadays, most of the surgeries performed in world surgical competitions are level 4 surgeries.
After all, if this wasn't a level 4 surgery, I'm afraid I wouldn't even get 120 points on this stage.
In fact, most neurosurgery operations are level 4 operations, including intracranial tumors, brain stem tumors, and some operations for cerebrovascular diseases, such as aneurysm clipping and vascular anastomosis, etc. These operations require the surgeon to perform careful operations, especially delicate operations under a microscope.
Well, looking at it this way, modern neurosurgery is actually divided into two categories, those requiring intervention and those under a microscope.
In the competition, microscopic surgery was the main feature.
The patient, a 23-year-old male, was diagnosed with a rare skull sarcoma. After surgery and multiple chemotherapy sessions, he unfortunately found that the cancer had recurred in January this year. He came to the hospital for further diagnosis and treatment.
This is the case selected by Dr. Huang Feng of the Chinese team.
Cranial sarcoma is also a rare case in neurosurgery. Huang Feng was obviously excited this time and wanted to give it a challenge.
It was really just a challenge. No one thought Huang Feng had ever done this kind of surgery in our hospital. At most, he had seen it. After all, this kind of surgery was usually discussed by the entire neurosurgery team, and the final surgeon must be at the director level. Everyone looked at Wu Xiaofu. He was really hurting people. Look, here is another one who dared to take risks.
Wu Xiaofu pretended not to see it and continued to look at the cases under Huang Feng's supervision.
Last February, the patient accidentally discovered a lump on the right forehead. At first, it was only the size of a peanut and hard in texture, so he did not take it seriously.
A few days later, seeing that the lump on the forehead continued to grow and was accompanied by pain, the patient's family immediately took him to the hospital for further examination. An MRI scan revealed a shadow on the skull, and the doctor highly suspected that it was a rare skull sarcoma.
Well, this is basically the common course of skull osteosarcoma. General osteosarcomas are basically treated by orthopedics, which has a special bone tumor department. However, if the skull is involved, the orthopedics department will take the initiative to let go and hand it over to the neurosurgery department.
Wu Xiaofu thought, fortunately he didn't study orthopedics, otherwise he would have to compete with neurosurgery for this job.
The diagnosis of osteosarcoma requires a combination of clinical, imaging and other auxiliary and pathological examinations. After osteosarcoma is highly suspected, it is necessary to further determine the nature of the patient's intracranial tumor. Generally, the pathology is sent for diagnosis after surgical resection of the space-occupying lesion.
The same was true for this patient, and the results suggested grade 2-3 osteosarcoma.
Grade 2 to 3 osteosarcoma is basically in the middle and late stages. Among tumors, osteosarcoma is highly malignant. Simple resection is not likely to suppress its development, and it must be supplemented with radiotherapy and chemotherapy.
In this case, the patient underwent 30 radiotherapy sessions after being diagnosed. Fortunately, osteosarcoma has a good specificity for radiotherapy, and the patient was followed up and no abnormalities were found.
However, this disease is a time bomb. Everyone knows that it is impossible to cure it completely. The only difference is how long it will take to relapse.
One year later, the patient underwent a 3D-CT head CT scan in the outpatient clinic, which indicated local skull and intracranial masses, and a high possibility of recurrence. This actually means recurrence. The imaging suggests a high possibility, but it is only because they are not a clinical department and do not have the right to diagnose.
However, when it comes to looking at films, the imaging department is more professional than the clinical department.
The initial surgery for skull sarcoma is very difficult, and performing another surgery after a recurrence is undoubtedly extremely difficult. If Huang Feng can succeed in the surgery, he will at least get 150 points.
Moreover, skull sarcoma is very rare and there is not enough case support. If you are not a senior director, you really wouldn’t dare to treat it.
So Wu Xiaofu endured the looks from Chu Jingming and the others. After all, Huang Feng was really taking a risk. However, since Huang Feng dared to do it, he should be confident about it.
Osteosarcoma is a malignant bone tumor with a low incidence rate. Data show that about 4 to 5 people per million develop the disease each year. And skull osteosarcoma, which occurs in the skull like this case, is extremely rare.
Well, with a one in ten million chance in front of him, if Wu Xiaofu were to do it, he would definitely not miss it.
"Because the surgical space in the brain is small and there are complex neural nuclei, the operation is extremely difficult because we need to completely remove the tumor while identifying and protecting these important structures."
……
As the operation began, more and more discussions arose. In group A, the case selected by Huang Feng was rare enough, which also led to more discussions.
Supratentorial craniotomy, resection of space-occupying lesions and cranioplasty were performed under general anesthesia.
Huang Feng has already determined the surgical method. After seeing it, everyone nodded. There is no controversy about this surgical method.
Under the microscope, the patient's lesions are shown very clearly, and whether a doctor has confidence in an operation cannot be hidden from other professional doctors.
The surgical thinking, surgical operation ability, and stability can all be seen at a glance.
Seeing Huang Feng's steady operation, Wu Xiaofu and the others felt relieved. Huang Feng was confident.
Wu Xiaofu is very interested in neurosurgery. After all, for the body, this field is the ultimate fantasy of doctors. If there is an opportunity, Wu Xiaofu might as well get involved in it.
There is also intervention.
Wu Xiaofu not only watched Huang Feng's surgery, but also other doctors' surgeries, including some interventional surgeries.
Interventional methods are becoming more and more popular now. They are used in interventional departments, general surgery departments, oncology departments, cardiology departments, and neurology and neurosurgery departments.
Because this is still an area that has not been covered by medical reform, for many departments, grasping intervention means grasping performance. As a result, many departments in the same hospital are competing with each other for patients. For example, some doctors are now performing interventional ablation of brain tumors.
Using small means to accomplish big things is what we call intervention.
Interventional ablation can really play a big role in liver lesions, especially tumors around blood vessels. Now, Wu Xiaofu is basically only lacking this area in hepatobiliary surgery.
After returning, Wu Xiaofu prepared to pick up this piece.
"Doctor Wu, there is a case for you to take a look at."
Ah!
Just when Wu Xiaofu was concentrating on watching the operation, a figure came up to Wu Xiaofu. Wu Xiaofu subconsciously took the medical record book, and then he found that the person in front of him was actually the director of a hospital in Stofford. Wu Xiaofu heard that others called him Professor James, who was also the big boss of general surgery.
"Professor James!"
Wu Xiaofu stood up and greeted him. Although this man was not the dean of the Stortford Hospital, in China, his status was only second to Rong Qiao. What's more, in terms of international influence, Professor James was probably even greater than Rong Qiao.
However, such a great professor actually asked him to help with medical records. This was too much of a respect for him.
Wen Xinhan and the others were also a little surprised. Has Wu Xiaofu reached this point? And even if Wu Xiaofu really has the ability, given the pride of the M country people, they would not ask Wu Xiaofu for help.
It was gallbladder cancer, involving the bile duct, with metastases to the pancreas, liver, and stomach. It was also a rapidly progressive type, and it took only three months from the onset of the disease to the progression to this extent.
This is basically a period of waiting for death to come. After all, for such terminally ill patients, there is basically no chance of surgery.
"We originally advised the patient to cherish this short period of time in the future, but the patient somehow heard about the surgery you performed in the competition, and then found me. He was willing to pay a huge price to ask you to perform a radical surgery using Coli No. 2."
That's it!
Wu Xiaofu understood something, and he asked why Professor James would look for him. It turned out that it was because of his wealth, and his status must be not low.
As for Coli No. 2, it costs one million yuan per injection and is in short supply. It can effectively curb the development of tumors. Of course, with the rapid progression of this pathological type, unless a radical surgery is performed to temporarily clear the lesions, otherwise, no matter how many injections are given, it will be useless.
However, it is really troublesome to perform surgery on this kind of people. It is not impossible for James to try the same surgery, but he did not do it. Instead, he found Wu Xiaofu. This in itself shows a problem.
He came to Country M to compete, so it's okay to simulate a patient, but it's better to stay away from cases like this.
"Wu, the patient said that as long as you are willing to perform surgery on him, the consultation fee is this much, and if the surgery is successful, the fee will be ten times higher."
hiss!
Wu Xiaofu was shocked, what a rich man!
